Modern Woodmen of America life insurance and annuity certificates will require a premium payment to maintain coverage unless indicated otherwise by the conditions of the contract. We strive to provide convenient payment options to our membership. There are three options from which to choose.
Monthly Automatic Bank Check Plan
Save time, money and postage by signing up for our Automatic Bank Check Plan. Your premium can automatically be deducted each month from a checking or savings account.
To sign up for this convenient payment plan, print, complete and return the
Automatic Bank Check Plan authorization form to the home office using the instructions provided in the PDF. To discontinue an ABC Plan, please contact your Modern Woodmen representative or the home office Service Center at (800) 447-9811, M - F, 8:00 a.m. - 4:30 p.m., Central Time.
Payment by Phone
Quarterly, semi-annual or annual premium payments may be made over the phone using our Tel-Debit option.* Simply call the home office Service Center at (800) 447-9811, M - F, 8:00 a.m. - 4:30 p.m., Central Time.
Information you will be asked to provide includes:
- Bank account holder's name
- Daytime telephone number
- Bank routing number
- Checking or savings account number

Mail
Certificates on a quarterly, semi-annual or annual mode will receive a premium due notice approximately two weeks before the premium is due. A pre-addressed return envelope will be mailed with the notices for your convenience in returning your payment by mail to the home office. The payment should be made by personal check or money order.** Please do not send cash.
